Patients with severe hepatic impairment (Child-Pugh B or above, hepatic or renal insufficiency, jaundice or hepatic failure) and patients on hemodialysis may require hemodialysis during the first 4 hrs of therapy. Hepatic impairment can cause withdrawal bleeding and withdrawal bleeding may occur during therapy with NEXium. Patients receiving hemodialysis should be monitored for signs and symptoms of autonomic neuropathy (irregular pulse, blood pressure, and heart rate). In some cases, the concomitant use of NEXium and benzyl alcohol may increase the hypotensive effect of NEXium, resulting in dizziness, lightheadedness, or fainting. Patients with a history of priapism (a prolonged and painful erection) and patients who have experienced infrequent or heavy loss of hearing or vision must not be treated with Nexium for at least 4 days after the first injection. If a penile erection persists following multiple penile injections, treatment with nitroglycerin should be continued for several days after the last injection. The patient should not use Nexium more than 4 hr before or four hr after the last injection. If the penile erection persists following multiple penile injections, treatment with Nitrates is recommended. Before initiating continuous intravenous infusion of NEXium, patients with a history of priapism (a prolonged and painful erection after a single injection) and patients who have experienced infrequent or heavy loss of hearing or vision should be monitored for signs and symptoms of autonomic neuropathy (irregular pulse, blood pressure, and heart rate).
